DIRECTIONS FOR APPLICATION FOR
FACULTY PROFESSIONAL DEVELOPMENT ASSIGNMENT

The application should include an original plus one copy of the following:

  1. A completed form entitled "Summary of Faculty Professional Development Assignment Application." This form is available at www.provost.iastate.edu/faculty/advancement/development/ or from the chair of your college professional development committee or from the office of your college dean.
  2. A more extensive document organized in the following way:
    1. A detailed description of the proposed program and the reasons for conducting it at the proposed site(s).
    2. A summary of your qualifications to undertake the proposed program.
    3. A description of how the program would benefit ISU and your professional development.
    4. A time-line of activities.
    5. A one-page report of productive use of any previous faculty improvement leave(s).
    6. Any additional comments pertinent to the application.
    7. Biographical information
      1. Educational background
        Degrees, dates conferred, granting institutions
      2. Academic experience
        List academic titles and dates of promotion
      3. Duties
        Give a synopsis of your current teaching, research, and service duties. Include a list of major departmental, college, and university committees on which you have served or are now serving.
      4. Honors
        1. List professional honorary societies.
        2. List offices held in professional societies.
        3. List awards received.
      5. Creative achievements
        1. Refereed publications
        2. Non-refereed publications
        3. Editorial responsibilities
        4. Recitals
        5. Exhibitions
        6. Critical reviews
        7. Others
  3. Append copies of correspondence (invitations, support, etc.) pertinent to this application.
Review Schedule
Sept. Your department will determine the date by which applications must be submitted for department review
Sept. 30 Ranked applications due in college dean's office
Oct. Applications ranked by the college committee due to college dean's office
Oct. 20 Two (2) copies of college-approved proposals are due in the Office of the Executive Vice President and Provost.

Please Note: A report on your activities will be requested by the Executive Vice President and Provost and due no later than October 1 of the year following your assignment.
